No, we are not currently able to accept paperwork online.

Your privacy and data security are our top priorities. While online intake forms are convenient, transmitting sensitive health histories and personal identification over the internet introduces unnecessary risks to data breaches.
To ensure your personal health information remains entirely secure, we require all new patient paperwork to be filled out by hand.

- Timing: Arrive about 20 minutes early for your consultation. If you have not finished your paperwork in advance, this will give you time to complete it in office.

- Dental insurance information
- A valid photo ID
- Medical history / list of current medications (medical clearance is required for full procedures)
- Preparation Note: You will be asked to remove any dentures, partials, or jewelry before your CT scan.
